Behavior Plan Performance Improvement Process General Questions

In May 2025, the Division provided information around the Behavior Plan Performance Improvement Project or BP PIP as a 5-year project the Division is working on with AHCCCS. This project is in its third year and is to address the backlog in overdue behavior plans for DDD members.

The Division requests that general questions and inquiries about behavior plans using this Google Form so inquiries and questions are properly tracked and addressed.

**Current Qualified Vendors who have questions about their specific member's behavior plan should contact the member's Support Coordinator directly
